What Runs in Our Family? Family health notes

Your notes stay on this phone or computer. Nothing is uploaded or sent anywhere. To keep a copy, use “Save & share”.

Health Stories: what we heard tonight

Scanned a card? You're on the right question. Write what people said, in their words. Always ask: who, how old, and which side of the family. Then add each person on the Family tree, and the summary for your doctor builds itself.

Family tree

Add each relative and anything they've had, with the age it started. “Healthy” and “don't know” are useful answers too. Start with parents, brothers and sisters: they matter most.

On a phone: tap “Print or save as PDF”, then choose Save as PDF (iPhone: Share, then Save to Files). Bring it to your next appointment or send it through your clinic's patient portal.

Save & share

Your notes live in this browser only. Clearing your browser data deletes them, so download a backup file now and then. To continue on another device, open the backup there.

This page is for keeping notes and sharing them with your health care team. It does not diagnose anything or estimate anyone's risk. The patterns it points out are ones that medical guidelines say are worth mentioning to a doctor.
