About this log

Two minutes, twice a day, seven days. Then one page your doctor can read in a minute.

One reading at the clinic says little: doctors want the average of many readings, taken at home, the right way. This log shows you how to measure so the numbers can be trusted, gives you a 7-day log and a monthly log, and turns it all into one page for your doctor.

Type into the fillable PDF and it does the sums for you: each day's average appears as you type, shaded in the color of its zone, then the 7-day average, the number of readings, the highest reading and the zone. Write your personal target from your doctor once and the log tells you whether your average is above it. The For my doctor page fills itself in, and one button prints or saves just that page. Prefer pen and paper? The print version has the same pages.

How it works

  1. MeasureSit, rest 5 minutes, and take 2 readings, 1 minute apart, every morning and evening for 7 days.
  2. LogType them into the fillable PDF, or write them on the printed page. Averages and the zone add up for you on screen.
  3. ShareTap Print / save for my doctor and bring the one-page summary, or send it, before your visit.

What you get

  • Start here: 8 habits for a reading you can trust, what your numbers mean (Normal, Elevated, Stage 1, Stage 2, Severe) and what to do at 180/120 or higher
  • Carol's first week: a filled-in example with notes on how to work out an average and what to do about a missed reading
  • My 7-day log: morning and evening, 2 readings each time, with arm, cuff size, monitor and your personal target from your doctor
  • My monthly log: one line a day for a whole month, with a medicine tick
  • For my doctor: your morning, evening and overall averages, highest and lowest, the zone, your medicines, symptoms and the questions you want to ask
  • My plan, at a glance: a fridge card with your target and your doctor's number, a before-each-reading checklist, a monitor and cuff check and your medicine times
  • Smart fillable PDF (Adobe Acrobat Reader): averages, zones and the doctor page fill in by themselves, and a Print / save for my doctor button prints only that page
  • A Simple edition with just the 7-day log and the doctor page, for anyone new to home readings
  • Every file in US Letter, A4 and 6 × 9, plus a Start Here guide

For tracking and sharing with your health care team. It does not diagnose or treat any condition.
