About this journal

Walk in ready. Walk out sure.

Most of what a doctor says is forgotten by the time you reach the car, and appointments are short. This large-print companion helps you write down what matters before the visit, ask what you need to in the room, and check what happens next: the new medicine, the test result, the follow-up.

It is made for patients and the people who love them. Grown children can fill in the first pages with a parent in ten minutes, join visits by phone, and get a photo of the summary box after each appointment. Twelve short visit skills explain the research behind each habit, with every claim graded by evidence.

How it works

  1. Two weeks beforeRate your main symptom each evening: 30 seconds.
  2. The day beforeWrite your three things, biggest first: 5 minutes.
  3. In the room and afterSay the plan back, put a date on every test, and send the summary to your helper.

What you get

  • A health card, care team, helpers and health care agent pages, and a medicine list with five spare copies
  • 18 visit kits of four pages each, ending in a summary box to photograph and send to family
  • 12 two-page visit skills: write it down, biggest worry first, bring every bottle, a second pair of ears, say it back, describe it clearly, medicines after 65, every fall counts, chase your results, after the hospital, checks that change with age, and say what matters most
  • Six filled-in example pages from Linda, 74, including a slip and a late phone call
  • Logs for medicine changes, test results, vaccines and checks; a leaving-the-hospital checklist and a once-a-year review
  • Guides to the words on medicine labels and common test numbers, an emergency page and a quick lookup
  • Free printable refill kits for visits 19 to 24 (yourbodydecoded.org/visit-kit)

A record-keeping companion, not medical advice: it gives no doses and names no brands.
